The Three-Peat form has been update on the Academic Advising website in the Forms section found under the Resources Tab: https://www.utsa.edu/advising/documents/forms/Three_Peat_Petition.pdf.

The new form no longer includes requests for information on graduation term or a semester by semester plan.

Please begin using the new form.
