The student and advisor will be emailed a scanned copy of the petition once a decision has been rendered.

Attached is a fillable PDF copy of the new Student Petition to Waive Additional Charges for Enrolling in a Course a 3^rd Time form.  Please note the student must submit the form and the 4 required documents directly to GSR 2.210, Dr. Martin’s Office, and the academic advisors signature is no longer required on this form.
