We strongly encourage your participation in providing feedback about our recent COEHD Dean candidates.  Attached is the feedback form in pdf and word format for you to complete and turn in by noon on April 26th.   There are a couple of ways to turn these in that can ensure your anonymity:

· You can print and send by campus mail to Vicki Weber/Senior Vice Provost/DT Campus. (You can either type the information before printing or write on the blank form).

· Scan and send to me electronically.  (I take all identifying information off the form and print it).

I want to assure you that I remove any identifying information as I compile the feedback.
