Hey Liz,
There are 2 options for these forms listed under the “Pre-Award Forms” section on the website. I have only had to provide them for a sub-recipient when they were an industry partner, non-FDP, and were not required to comply with 2 CFR 200
audit requirements. This form is used for financial risk assessment.
