[Advising] Orientation Appointment Form

Hello Everyone: Please remember if DegreeWorks is down to use the Orientation Appointment Form to document the course recommendations and notes for your advising appointments. The completed form will need to be scanned and uploaded into SSC-Campus. Also, make sure the student leaves the appointment with a copy of the form.
