Dear Colleagues, In anticipation of Dean Torres' start date (8/2/21) and to ensure that agreements are signed by the appropriate authority, please use the revised routing form when sending affiliation/program/cooperation agreements etc. beginning 8/1/21. After completing this form, send to chair/director to sign. Then send completed form, agreement, & legal approval to Associate Dean. If it is a renewal, then also attach prior agreement documentation. UG = Flores Grade = Langman AD then forwards after reviewing. Please note that this process may change because agreements will soon be routed through Cayuse. Until further notice, use this process. Thanks!
